Phase 3 trials have read out; the approval application is not yet filed in United States. Eli Lilly's TRIUMPH programme has now read out in full. TRIUMPH-1 (pivotal, 2,339 adults): up to 28.3% body weight, the highest figure recorded for a single obesity medicine. TRIUMPH-4 (with knee osteoarthritis): 28.7% at 68 weeks. On 23 July 2026 the last two reported: TRIUMPH-2 (1,152 adults with obesity and type 2 diabetes) 12.7% to 20.8% at 80 weeks versus 4.0% on placebo, with A1C down up to 1.6 points; and TRIUMPH-3 (1,949 adults with severe obesity and established cardiovascular disease) 21.6% to 22.6% at 80 weeks versus 3.2% on placebo, plus improvements in lipids, blood pressure, hsCRP and waist. Lilly announced it will file for approval in the first quarter of 2027. In June 2026 the TRIUMPH-1 baskets were presented: knee osteoarthritis pain −73.1% (4.3 points; −75.8% in TRIUMPH-4, the dedicated trial) and moderate-to-severe sleep apnea −60.6% (36 fewer events per hour); TRIUMPH-1 was published in NEJM (June 2026, ADA) and on 29 September 2026 TRIUMPH-2 was published in full in The Lancet (EASD 2026). In type 2 diabetes without obesity, TRANSCEND-T2D-1 read out in March 2026 and was published in full in The Lancet in June 2026. The dedicated cardiovascular trial (TRIUMPH-Outcomes) reports around 2028–2029. What the class has shown and is still unmeasured here: heart and kidney (TRIUMPH-Outcomes, primary completion February 2029) and liver inflammation. Next step: filing the approval application.

Completing phase 3 is NOT approval. Retatrutide is not approved by the FDA or any other agency, and will not be until 2027 or 2028 if the filing succeeds. It remains an investigational compound. Lilly, on its own page (September 2026), states that it is not approved by any agency and that it is distributed only inside its trials. What CAN be checked in a vial, by independent analysis (HPLC and mass spectrometry), is its identity, purity and content: that is why every lot of ours carries a verifiable certificate of analysis. What no analysis can give is clinical approval: it remains research-use-only material.

Questions

Frequently asked questions

What is it reconstituted with, and how much?

With bacteriostatic water: 1 ml into the 10 mg vial, giving 10 mg/ml.

What is the starting dose of the research protocol?

1.5 → 3 mg, 1×/semana. This is the SBL research protocol, with no established clinical regimen in people.

How long is the cycle, and how long does a vial last?

16 weeks. At the starting dose, the 10 mg vial lasts ~3 sem.

Which route is used in research?

Subcutaneous.

When does it make no sense?

It makes no sense with: Personal or family history of medullary thyroid cancer or MEN2, Active cancer or oncology treatment in the last 5 years, Pregnancy or breastfeeding, Gastroparesis or slow gastric emptying, Active eating disorder. Details in the Safety tab.

What is it combined with?

It goes well with MOTS-c. The reasons are in the Protocols tab.

How do I check my batch?

With the code on the vial, in the verifier. The CoA certifies the identity and purity of the batch, not safety of use.
