Technical description.
Cagrilintide is a long-acting amylin analog. Amylin leaves the pancreas together with insulin and tells the brain you have eaten: it slows stomach emptying and brings satiety forward.
SuperBio Labs Cagrilintide is supplied as lyophilized powder with HPLC purity ≥99%, with a Certificate of Analysis per lot.
It works through a different pathway from GLP-1s, which is why it was studied with semaglutide. In REDEFINE 1 (3,417 adults, 68 weeks), on its own it lowered weight 11.5% and with semaglutide 20.4%, versus 14.9% for semaglutide alone and 3.0% for placebo (11.8%, 22.7%, 16.1% and 2.3% in those who stayed on treatment). In type 2 diabetes the combination lost 13.7% (REDEFINE 2). Against tirzepatide (REDEFINE 4, 84 weeks, open-label) the combination lost 20.2% vs 23.6% and did not reach non-inferiority. The FDA has been reviewing CagriSema since 18 December 2025; Novo Nordisk guides to a decision in late 2026.

Honest side effects.
The digestive profile of its class. Nausea, constipation and vomiting are the most common, mostly during dose escalation. With semaglutide in REDEFINE 1: nausea 55%, constipation 31%, vomiting 26% (placebo 13%, 12% and 4%).
Alone it is milder than the combination. Discontinuations due to adverse events with the combination: 5.9% (placebo 3.5%). Others: fatigue, reflux. In type 2 diabetes (REDEFINE 2) discontinuations due to adverse events were 8.4% versus 3% on placebo. Two class notices: if you are having surgery or an endoscopy under sedation, say you use this kind of compound (slow gastric emptying; FDA label, Nov 2024). If you vomit or have diarrhea, hydrate: dehydration can harm the kidney. Do not use in pregnancy or with prior pancreatitis. If you already use a GLP-1, that pairing is what was studied, but add it with guidance.
